How can I register to use the VA Medical Center in Fayetteville and its outpatient facilities?
Veterans can use the VA Medical Center Fayetteville after submitting a 1010EZ application form along with a copy of their DD 214. The veteran must have received a discharge that is other than dishonorable to qualify for medical care. The application for enrollment can be completed online or veterans can also visit the Jacksonville Community Based Outpatient Clinic (CBOC) located at:

At the clinic the veteran can enroll by bringing in their DD214, their VA disability rating letter if they have one or their income information if they do not have a service connected disability. The CBOC can be reached at 910-353-6406.
