Required School Vaccinations

North Carolina law requires all children in the state to receive certain immunizations. The School Immunization Packet (PDF) provides extensive information on school vaccinations.

To have your child vaccinated in their school by the Health Department, please complete the School Initiative Immunization Outreach Form (PDF) and turn it in to your child's school.
