1. Who qualifies for CAP/DA?

The ones who qualify for CAP/DA are those who:
- Are 18 years of age or over
- Live in Onslow County
- Deemed appropriate for basic nursing home level of care
- Are at risk for nursing home placement without assistance of community based care
- Meet financial eligibility criteria
- Can have their safety and well being maintained in the home within the Medicaid cost limits
- Have an emergency backup plan formal and informal support
- Meet the two waiver service criteria

2. What types of services are provided?

The types of services provided by CAP/DA are:
- Comprehensive interdisciplinary needs assessment by social worker and nurse
- Extensive list of waiver services and other state plan service through a distinct plan of care
- Personal care aide
- Personal Emergency Response Services
- Home accessibility and adaptation
- Specialized medical equipment and supplies

Case management services are provided such as:
- Assessing
- Care planning
- Monitoring and follow-up
- Referral/linkage

3. How do I apply?

To apply for CAP/DA services you can call 910-455-2747 and speak to any of the CAP/DA staff. When calling to apply be prepared to give basic information about medical / health needs as well as income and insurance information.

Also, we are happy to take referrals from individuals acting on behalf of family members or beneficiaries.

4. Is there a waiting list?

Yes there is a waiting list, however you are encouraged to apply. The waiting list is on a first-come, first-served basis.
