Physical/ pap smear with birth control methods is provided through our Family Planning Program. Services provided through the Family Planning Program are based on a sliding fee scale that takes into consideration your economic unit income and family size. We would need income information of those providing financial support to you. We typically look at income information for the past year.
If unemployed at the time of your appointment, we will look at income for the past 6 months and project zero income for you 6 months into the future.
If you have Blue Cross Blue Shield coverage at the time of your appointment, we can file BCBS first. Any remaining unpaid balance will be your responsibility based on SFS from 0% - 100%.
