What To Expect

This program provides home visits within the first 28 days after hospital delivery to ease the transition to home for both mother and baby. Health history of mom and baby, newborn physical assessment, education & referrals as needed. 

Services provided include:

  • Assessment of newborn including weight
  • Assessment of postpartum mother
  • Home visit by a Registered Nurse
  • Lactation information and support
  • Nutrition support for mother and baby
  • Postpartum depression screen for mother
  • Referrals to other services within the community

Participate in a Home Visit

The Community Transition Coordinator will contact the mother at Onslow Memorial Hospital after delivery. Parents who participate in Baby Love Insurance Program or any other interested parent are eligible for this service.
