How to access your patient portal:

You can sign up now for the benefits of the patient portal.  If you're 18 years or older.

When you visit the health department for your appointment, health department staff will start the process of registering.  You must sign a consent form to gain access to your patient portal.

If you haven't signed a consent form and would like to register for the patient portal, please visit our office and staff will register you to gain access.

You can finish setting up your account with a password that will be emailed to you.  Be sure to check your spam/junk folder if it doesn't reach your inbox.

Cuando visite el departamento de salud para su cita, el personal del departamento de salud comenzará el proceso de registro. Debe firmar un formulario de consentimiento para obtener acceso a su portal de paciente.

Si no ha firmado un formulario de consentimiento y desea registrarse en el portal del paciente, visite nuestra oficina y el personal lo registrará para obtener acceso.

Puede terminar de configurar su cuenta con una contraseña que se le enviará por correo electrónico. Asegúrese de revisar su carpeta de spam/basura si no llega a su bandeja de entrada.
